Angus

Stakeholders in Angus are currently putting together a project to employ a member of staff who will develop an Angus wide palliative care strategy that gives pathways and resources for all sectors.  For further information, contact ivan.cornford@scottishcare.org

At present care homes are able to contact community nursing services directly in palliative and end of life care rather than going through other more indirect systems - NHS 24 etc. 


Perth & Kinross

Cornhill is the main centre in Perth& Kinross Health & Social Care Partnership for specialist palliative care services including in-patients (10 beds), Community Macmillan Nurses who are aligned to GP practices (and part of wider primary care teams) and the Hospital Palliative Care Team which supports the wards in Perth Royal Infirmary with people with palliative and end of life care needs. 

The GPs and District Nurses, whilst not specialist, are core to the provision of palliative care for people living in the community and have strong links with Marie Curie Services.

The District Nursing teams provides a "fast track" service supported by Healthcare Support Workers to people who require support to prevent hospital admission, to support discharge from hospital at end of life and to support carers with crisis situations.
